Achievements
This page lists the achievements obtainable in Final Fantasy XIV. The list is complete up to version 3.2.
Each achievement gives you a certain number of points that add up to your grand total, making up a rough numerical value of how much of the game you've experienced. The total comes up to 10,165 points from A Realm Reborn only, and an additional 1,800 points from Legacy achievements1, for an absolute maximum of 11,965 points. The total points achievable in Legacy version 1.23b of the game was 4,255.2

Other than the ones listed above, version 1.x of the game separated the ARR achievement category Items into Items, Currency and Materia, ARR Battle into Battle and Dungeons, and the ARR Quests category into Quests and Seasonal Achievements.
Rewards
Aside from giving achievement points, there are various achievements that give other rewards as well, such as titles for your character and equipment. These are not delivered to you automatically when you complete an achievement with a reward, you must instead visit Jonathas at Apkallu's Falls in Old Gridania.
Unlocking (1.x)
This section features info only relevant to version 1.x of the game. It is preserved here for archival purposes.
To be able to start gathering your achievements, you must unlock each category separately. Some achievements (the ones which have History entries on Lodestone for example) backtrack their progress from before you unlock the category for it, but do note that some do not track and you can only start making progress for them after unlocking the appropriate category.
To obtain any reward from an unlocked achievement you must visit the same person who unlocked the category of the achievement you gain the item from. You can find a list of these people and their locations below.

Trophies
Aside from having the in-game achievements listed above, the PS3 and PS4 versions of the game have a list of trophies to obtain. They number 39 in total, and consist of 1 platinum, 3 gold, 15 silver, and 20 bronze.
Note that if you play on several different platforms, the PSN does not recognize anything you did on another platform (such as PC). All trophy goals must be achieved on a PS console for it to register.
